There are 5 parts to the DOT Chain of
Custody forms, and each part must be filled out by a different party in the drug
test process. Truck drivers, aspiring boat captains & others would be subject to
the same processes.
Employer Makes the Request with the proper required and approved form.
The Collector will watch the driver
before and after the sample is given for any suspicious behavior, and must note
anything they see down on the Chain of Custody form. Once they have the sample in
their possession, they check the temperature of the sample as well as volume,
coloring, and odor, and note down if they find any abnormalities. If they note
anything strange with the sample, it will be sent to a more sophisticated facility
for testing.
Before the sample is sent off to a testing facility, the donor must watch the
collector pour their sample from the collection container into a specimen bottle and
seal it. They’ll then give the donor a certification statement which must be read
and signed. The sample will then be sealed and shipped to the testing facility.
The sample arrives at the testing
facility/Lab, where it is tested. Once testing is completed, the Chain of Custody
form is sent to the MRO. The Lab does not relay the results, but simply does the
testing.
The MRO receives the completed Chain
of Custody form and examines it for any flaws. They then sign off the results and
the process is complete.
The term "Chain of Custody" refers to the comprehensive documentation, either in paper or electronic form, that meticulously records the sequence of custody, transfer, control, analysis, and eventual disposal of any physical or electronic evidence. Although frequently associated with drug testing, as exemplified by the DOT Chain of Custody form provided earlier, Chain of Custody documentation finds application across various domains including supply chain management and legal proceedings such as criminal cases.
The Chain of Custody Form meticulously records all individuals involved in the handling of a drug or alcohol specimen. In the transportation sector, this form is particularly crucial for drug testing new drivers. The Department of Transportation (DOT) introduced this form to comply with the Mandatory Guidelines for Federal Workplace Drug Testing Programs, ensuring standardized procedures across the industry.
The Chain of Custody Form serves as a tool to monitor every individual who interacts with and holds a sample from the moment of collection to analysis. When employing the DOT drug test Chain of Custody form, the risk of false positives and false negatives diminishes significantly due to the heightened difficulty in tampering with the sample.
Sample Collection
The donor (let's say truck driver) produces their urine sample (at an approved collection site determined by your employer or a referral from your employer) and it is documented when it was taken on their copy of the Chain of Custody form. This sample is taken in a controlled approved environment because the bathroom must meet specific mandated requirements. The Collector location (not Substance Abuse Consultants) personnel- once they have the sample in their possession, they check the temperature of the sample as well as volume, coloring, and odor, and note down if they find any abnormalities. If they note anything strange with the sample on the C.of.C form.
Prior to dispatching the sample to a testing facility, the donor must observe the collector as they transfer the sample from the collection container into a specimen bottle and securely seal it. Subsequently, the collector provides the donor with a certification statement, which the donor must thoroughly read and sign. Following this, the sample is sealed and dispatched to the designated testing lab facility partners determined by your employer and relayed to us.

Lab Delivery & Testing
The sample arrives at the testing facility, where it
is tested. Once testing is completed, we are notified of the results and the Chain of Custody
form is sent to us to review details. Upon receipt of the completed Chain of Custody form, the
MRO meticulously reviews it for any discrepancies or irregularities. Our medical review officers
will analyze the results and CofC, verify details, sign off on negatives, or contact donor &
employer for any positives.
After the officer has authorized the results by
signing off on the form, it is logged in our client portal, the employer client gets an email
notification they can login and review the results marking the conclusion of the
process.
